A part of you sets out on a familiar journey. A series of rapid blurs prefaced your third eye's slow birth into a sizable room. It was twice the size of standard sized from from North to South. The room was dark and dank, cave-like. And a misty vapor seemed to swirl through the air.
One door exited to the East; it was situated at the northern end of the East wall. Another door was straight across from it exiting West, situated into the northern end of the West wall. The final exit to the room was set into the middle of the Southern wall.
Feeding from the middle of the Northern wall, a bone dry canal extends southward into the room, but then curves East to feed into the Eastern wall, just down from the door there. After making note of that, you were able to drift to the center of the slick grotto.
Along the Eastern face of the wall were intricately carved faces. Many, many faces. They all stared ahead with dead eyes to the Western wall of the room. Some were intact, some were damaged and missing chunks of their chiseled visages. One or two seemed to have eroded completely. But there were plenty remaining.
On the Western wall face was a seemingly never ending cascade of water. It filtered in somewhere along the top of the cave wall, and appeared to evaporate into the swirling mist that filled the room as it splattered upon the floor below. It was a very primitive setting, and you could not stop staring at all of the faces of varying size and expression.
It was as if the network of dead stone eyes watched you even now, in your ethereal form exploring the room. For once you decided it was time to be done with this, and shook yourself from the vision on your own.

Right away your mind is swept away to another room. It was a standard sized room, with exits leading to the East and to the West. It was an unremarkable room, with metal walls and flooring. In the middle of the room sat a sizable wooden crate with a closed lid. There was black lettering diagonally across the front of the crate, but some of the letters had rubbed off. You could make out the letters P, L, and S.
Due to the lack of confusion in the layout, you were able to quickly memorize the notes and shake yourself free from the vision.

This is going to completely depend on how far back the reset actually goes in the phase apparently. I really hope I haven't wasted a scan on this. :\ | A hero is no braver than an ordinary man,

Your mind drifts to a long room with chains holding up a platform. You remember this one. It's three times the length of a normal room with five exits. Two East and West on the Northern side, two East and West on the Southern side and one North. The one of the South-Eastern side is locked. The chains sway gently but nobody is around in here.

Your mind's eye takes you away from here to another place. The shape of the room is very strange. A space the size of a standard room is situated to the West with a single exit on the Western wall. The room then continues East for the width of another standard sized room ending with an exit on the Eastern wall. Finally, moving South from this position there is another standard sized room space with an exit against the Southern wall.
The room itself is full of sand and soil, scattered around in awkward positions. The ground is very uneven and unsteady looking in places. By each of the exits, there is a wooden sign. The sign contains an image of a skull and crossbones. These images are facing away from the doors however, so they cannot be seen as you first walk through.

Your mind's eye drifts into a standard sized room. There are exits to the East and West. The North side of the room contains a large heavyset metal door. The door has a red light on it and a ring of black paint. Beneath the door are a set of tracks, leading behind it.
The room itself is dimly lit. The floor is solid metal and the wall to the North has a small alcove in it. Something is glowing beyond the alcove though. Two small points of red light that almost look like eyes. They are watching the room beyond menacingly.
