Either one would make me incredibly paranoid, but I'd rather know the date. If I knew the cause, every waking moment would be spent worrying that the cause was just around the corner. With the date, you may have the impending doom rapidly approaching on the horizon, which would be stressful, but at least you'd KNOW when it'd happen and could try to enjoy life while you can.

The date. Then I know that I'm safe until then! |
Unless it was tomorrow.
INTERESTING SIDE THOUGHT EXPERIMENT:
If you were guaranteed to die soundlessly in your sleep and it was completely assured that it would happen that way, that it would be painless and you wouldn't feel a thing would this be good or bad? You don't know when, just that you won't know about it at all.
On the one hand you'd be anxious every night but on the other you'd have some pretty fulfilled days. Not sure if that would be good or bad really. |
I'd want that to happen, but I wouldn't want to KNOW it'd happen. At all.

If I knew how I'd die and it turned out I would be murdered by a clown with a scalpel, I wouldn't fucking leave the house until he would come and get me. |
Yeah. If you knew the method, you could just do everything to avoid said method. |

Your death cannot be prevented from this knowledge at all. |
